This sconce is model cn112 A4 from brass light Gallery in Milwaukee Wisconsin. It is the curved arm 2-1/4 inch shade holder wall sconce light. It is the Retro model, which is a registered trademark of the brass light Gallery in Milwaukee. This listing also includes a discontinued shade for this sconce which is opal gloss glass about 6 and 1/4 inches tall and the opening at the top is 4 and 1/4 inches in diameter. A hairline split is visible above one of the three set screws that secures the shade, as shown in the photo. This seems to be a design element, since it it present on both sconces.
